Health Improvement Coordinator (Full Time)

Physicians CareConnection, the largest coordinator of voluntary health care and social services in Central Ohio and locally recognized for its unique and extraordinary model of coaching and coordinating services, is actively seeking a Health Improvement Coordinator to join our dynamic team.

We use a coaching approach at Physicians CareConnection which means we recognize the complexity of people’s lives. We continually strive to understand the impacts of institutional racism, the implicit bias within organizations and systems, and how those factors influence our work to develop and deliver services.

We are seeking talented people with the following skills and experience:

1. Listening and empathy: When faced with a challenging situation, do you listen or ask questions about others’ feelings, thoughts, or needs?

2. Collaborative and transparent relationships: Do you appreciate and recognize the contributions of coworkers and clients and communicate a willingness to adapt based on the situation?

3. Respect and humility: Do you recognize the power and self-efficacy of individuals to lead the change and drive the solutions in their own lives?

4. Flexibility and adaptability: Do you recognize that managing change is complex and each person is different and a fluidity in approach is needed to solve problems and support individuals?

5. Recognizes the impacts of inequities on peoples’ lives: Do you have a non-judgmental attitude towards clients and coworkers; and demonstrate critical thinking and awareness of how power and privilege shape individual and organizational service delivery?
